Hospital referral of detainees during police custody in Amsterdam
This study describes how many detainees have been referred to emergency departments for further evaluation or emergency care while in police custody in Amsterdam (years 2012/2013). It provides insights into the diagnoses assigned by forensic doctors and hospital specialists and the appropriateness of the referrals. We made use of the electronic registration system of the Forensic Medicine Department of the Public Health Service Amsterdam. This department is in charge of the medical care for detainees in the Amsterdam region. Hospital diagnoses were obtained through collaboration with several Amsterdam-based hospitals.
According to our results, in 1.5% of all consultations performed, the detainee was referred to hospital. The most frequent reasons for referral were injuries (66%), intoxication/withdrawal (11%) and cardiac problems (7%). In 18% of all referrals, hospital admission (defined as at least one night in the hospital) was the consequence. After review of hospital files, the indication for referral as stated by the forensic physician was confirmed in 77% of all cases. A minority of referrals was considered unnecessary (7%). The identified cases allow for a discussion of cases of over-referral. Future research should focus on the problem of under-referral and associated health risks. -

Abstract: In this study, the data of 7208 victims (children and adults) of domestic and public violence were analysed after they reported this to the police in Amsterdam, the Netherlands. In this analysis the characteristics of these intentional injuries were collected and compared. Despite some significant differences, there is no clear, specific way to distinguish between public and domestic violence.
Therefore, it is more efficient for doctors to limit their focus to the differences between accidental and intentional injuries.Reijnders, U. J. L., & Ceelen, M. Signalering van lichamelijke mishandeling
Inleiding: Huisartsen spelen een belangrijke rol bij het signaleren van lichamelijk letsel dat is veroorzaakt door huiselijk geweld. Doel van dit onderzoek was om de ervaringen van huisartsen op dit gebied te inventariseren.
Methode: We schreven 412 zelfstandig gevestigde Amsterdamse huisartsen aan met het verzoek een vragenlijst over de signalering van lichamelijk geweld in te vullen. We kregen 251 bruikbare vragenlijsten terug (respons 61%).
Resultaten: Vijfenzestig procent van de huisartsen gaf aan dat er gedurende de huisartsenopleiding aandacht is besteed aan de signalering van lichamelijke mishandeling; 64% van de huisartsen heeft na- en/of bijscholing gevolgd op dit gebied. Scholing en karakteristieken van de huisarts bleken belangrijke factoren die van invloed zijn op het aantal vermoedens van lichamelijke mishandeling. Bijna driekwart van de huisartsen gaf aan belemmeringen en/of beperkingen te ervaren bij het signaleren van lichamelijke mishandeling. Het merendeel van de huisartsen stond positief tegenover het idee huisartsen de mogelijkheid te bieden vermoedens van lichamelijke mishandeling direct te bespreken met een forensisch arts (70%).
Conclusie: Een groot deel van de huisartsen ervaart belemmeringen bij het signaleren van lichamelijke mishandeling. Scholing en consultatie van een forensisch arts zouden kunnen worden ingezet om ervoor te zorgen dat huisartsen beter in staat zijn om lichamelijke mishandeling te signaleren.Ceelen, M., Dorn, T., Buster, M., de Keijzer, K., & Reijnders, U.
